In Kenya, where I live and work, malaria still places an unacceptably high burden on families and communities, with approximately one-third of the population living in high-risk transmission zones, particularly lake endemic regions of western Kenya.

The disease continues to disproportionately affect children under five years of age and pregnant women, contributing to high morbidity, mortality, and economic losses. The Kenya Malaria Strategy 2023–2027 recognizes malaria as a major impediment to health and development, while also acknowledging the substantial progress made in recent years.

Over the past two decades, Kenya has achieved important gains through the scale-up of evidence-based interventions, including insecticide-treated nets (ITNs), indoor residual spraying (IRS), improved diagnosis, and effective treatment with artemisinin-based combination therapies. Importantly, the progress in Kenya reflects broader advances across the African continent, where sustained malaria interventions have saved millions of lives. However, as highlighted in recent WHO reports, the burden remains unacceptably high, underscoring the need to sustain momentum and accelerate efforts towards elimination.

What will it take to reach zero malaria in Kenya and sub-Saharan Africa?

Ending malaria will require sustained political commitment, increased domestic financing, stronger health systems, and continued investment in research and innovation. Kenya’s malaria strategy emphasizes optimizing coverage of high-impact interventions, strengthening surveillance systems, and accelerating the adoption of innovative tools and technologies. Achieving elimination will also require addressing challenges such as insecticide resistance, antimalarial drug resistance, climate-related changes in transmission patterns, and change of the historical malaria landscapes.

In order to accelerate progress, it is important that countries harness emerging innovations, technologies and strengthen data-driven decision-making. Recent advances, including malaria vaccines, genomic surveillance, improved vector control tools, and digital health technologies, have the potential to transform malaria control and elimination efforts. At the same time, stronger regional and cross border collaboration will be essential.

WHO’s World Malaria Report 2025 highlights that while substantial gains have been achieved globally, progress towards the 2030 targets remains off track. To reverse this trend and move towards elimination, countries must combine scientific innovation with resilient health systems, community ownership, and sustainable financing mechanisms.

Damaris Matoke-Muhia, Senior Principal Research Scientist, Kenya Medical Research Institute (KEMRI). Their success reflects the consistent implementation of a comprehensive package of evidence-based interventions, supported by resilient health systems and long-term financing.

A key lesson is the importance of high-quality surveillance systems capable of rapidly detecting, investigating, and responding to every malaria case. As countries move from malaria control to elimination, surveillance becomes a core intervention, enabling programmes to identify transmission foci and deploy targeted responses to prevent onward transmission.

These countries also demonstrate the value of data-driven and targeted approaches. The WHO recommends that countries approaching elimination tailor interventions to local transmission patterns through targeted and reactive strategies, including case investigation, reactive case detection, and other focused responses that address residual transmission.

KEMRI’s molecular entomology team in the laboratory. Communities play a critical role in prevention, prompt care-seeking, adherence to interventions, and supporting surveillance activities. The WHO also emphasizes community engagement as a key determinant of success for elimination programmes.

Finally, it is important to note that elimination is not the end of the journey. These countries have maintained robust surveillance and rapid response systems to prevent the re-establishment of transmission from imported cases. Their experiences show that with strong leadership, effective surveillance, community participation, and sustained investment, malaria elimination is an attainable goal for other malaria-endemic countries in Africa.

Damaris Matoke-Muhia, Senior Principal Research Scientist, Kenya Medical Research Institute (KEMRI). I believe the greatest gains have come from combining effective vector control with prompt, effective case management. The widespread use of insecticide-treated nets, indoor residual spraying, improved diagnostics, and treatment with Artemisinin-based Combination Therapies (ACTs) has contributed significantly to reducing malaria-related illness and deaths, particularly among children under five years of age and pregnant women.

These efforts have contributed to reductions in malaria prevalence and mortality across the disease epidemiological zones. Kenya’s malaria strategy aims to build on these achievements by reducing malaria incidence by 80% and malaria deaths by 90% from 2023 levels by 2027/28, while also interrupting indigenous transmission in selected counties.

At the Kenya Medical Research Institute (KEMRI), I have had the privilege of contributing to and witnessing research that has shaped malaria control policies and practice. This includes studies on insecticide-treated nets, insecticide resistance monitoring, antimalarial drug efficacy, and malaria vaccine development. I also contributed to KEMRI’s spatial repellent research, which generated evidence that informed WHO recommendations and Kenya’s Spatial Repellent Guidelines, demonstrating the potential of spatial repellents as a complementary malaria vector-control tool. KEMRI has also contributed to clinical trials of antimalarial medicines and the RTS,S malaria vaccine, helping generate evidence for national and global policy decisions.

Despite these achievements, challenges remain, including insecticide and drug resistance, funding constraints, climate change, and changing mosquito behavior. However, I remain optimistic. Emerging innovations – including next-generation nets, spatial repellents, house-improvement interventions, ivermectin-based approaches, malaria vaccines, genomics, and gene drive technologies – offer renewed hope that malaria elimination is achievable in Kenya, across Africa, and globally.

World Mosquito Day is a time to reaffirm commitments, strengthen collaborations, and reflect on what is needed to move towards a world free of malaria.
